Comparison between Endoscopic Septoplasty and Conventional Septoplasty: Our Experience

[Year:2015] [Month:September-December] [Volume:8] [Number:3] [Pages:6] [Pages No:93 - 98]

Keywords: Conventional septoplasty,Deviated nasal septum,Endoscopic septoplasty,Septoplasty

Abstract

Introduction

Nasal obstruction due to deviated nasal septum is a common problem encountered by otolaryngologists. A variety of surgical procedures have been tried in the treatment of the same.

Objective

This study was conducted to evaluate the outcomes and complications of endoscopic and conventional septoplasty.

Materials and methods

This is a prospective, randomized study done in Government Medical College, Latur in which 52 patients with symptomatic deviated nasal septum were included in the study. Eighteen of them underwent conventional septoplasty and the rest had endoscopic septoplasty.

Results

The difference in the functional outcome of both the surgeries was insignificant. Endoscopic septoplasty had better outcome with respect to complications and proved as an easier method to correct posterior deviations and isolated spurs.

Conclusion

Endoscopic septoplasty allows accurate, conservative repair of obstructive nasal septum deviations, with fewer complications and better functional results compared to conventional septoplasty.

Endoscopic Trans-ethmo-sphenoidal Optic Nerve Decompression for Traumatic Optic Neuropathy: Case Series and Review

[Year:2015] [Month:September-December] [Volume:8] [Number:3] [Pages:5] [Pages No:99 - 103]

Keywords: Blindness acquired,Endonasal trans-sphenoidal approach,Endoscope,Methylprednisolone,Optic canal decompression,Traumatic optic neuropathy

DOI: 10.5005/jp-journals-10013-1243  |  Open Access |  How to cite  |

Abstract

Objective

To assess the visual outcome after endoscopic, endonasal trans-ethmo-sphenoidal optic nerve decompression in patients with traumatic optic neuropathy.

Materials and methods

Prospective study, at tertiary referral center. Study included 12 patients with post-traumatic indirect unilateral optic nerve injury, not responding to medical management with methylprednisolone. These patients underwent optic nerve decompression via trans-ethmo-sphenoidal route using nasal endoscope. Main outcome measure was postoperative visual acuity.

Results

Out of the 12 cases of traumatic optic neuropathy (TON) operated upon, nine had good visual outcome.

Conclusion

The endoscopic endonasal trans-sphenoidal approach can decompress the traumatized and swollen optic nerve with excellent visualization of the orbital apex and optic canal, without the morbidity associated with brain retraction as in the conventional transcranial decompression.

Influence of Superior Attachment of Uncinate Process on Presence of Agger Nasi

[Year:2015] [Month:September-December] [Volume:8] [Number:3] [Pages:3] [Pages No:104 - 106]

Keywords: Agger nasi,Frontal sinus,Superior attachment of uncinate process

DOI: 10.5005/jp-journals-10013-1244  |  Open Access |  How to cite  |

Abstract

Objective

The superior attachment of uncinate process and agger nasi (AN) cell are important landmark in frontal sinus surgery, and are in close relation to each other. The aim of the study is to investigate the relationship between the superior attachment of uncinate process (UP) and presence of AN cell.

Study design

A retrospective radiological study.

Materials and methods

Fifty patients with features of chronic sinusitis were evaluated radiographically, i.e. computed tomo graphy (CT), 100 sides, i.e. 50 scans were evaluated for superior attachment of UP and presence of AN. Chisquare test was used as statistical tool for eliciting the relationship between the two.

Results

The AN cell was found in 68 (68%) of 100 sides. The prevalence of AN cell according to superior attachment of UP types were 64.1% for lamina papyracea, 79.41% for skull base, 53,84% for middle turbinate. The presence of AN cell accor ding to superior attachment types of UP was not statistically significant (χ2 = 3.5925, p = 0.16592).

Conclusion

The AN cell found in 68 sides (68%), and there was no statistical significance between the superior attachment of UP and presence of AN cell.

Abstract

Results

Out of 576 patients, 117 patients were positive for SPT for various inhalant allergens, with prevalence of comorbid asthma being 41.08%. There was significant improvement in symptomology of asthma following the use of nasal inhalational steroids for AR. Wilsons score system and spirometric parameters showed marked improvement in post-treatment results, with p-value of 0.00 which is statistically highly significant.

A Bunch of Stapler Pin: An Unusual Nasopharyngeal Foreign Body

[Year:2015] [Month:September-December] [Volume:8] [Number:3] [Pages:3] [Pages No:127 - 129]

Keywords: Foreign body,Nasopharynx,Stapler pin,Unusual site

DOI: 10.5005/jp-journals-10013-1250  |  Open Access |  How to cite  |

Abstract

Objective

To highlight the occurrence of unusual foreign bodies in the nasopharynx in the pediatric age group.

Introduction

Foreign bodies are common in ENT practice universally. At times they may present as emergency requiring urgent intervention and many a times they go unnoticed as these are not suspected. Nasopharyngeal foreign bodies are rare in any age group.

Case Report

A 10 months old female child was brought with the history of persistent drooling of saliva and refusal to feed after accidental ingestion of a bunch of stapler pin. Plain radiograph showed a bunch of stapler pin in the nasopharynx with enlarged adenoid tissue and prevertebral soft tissue shadow.

Conclusion

Bunch of stapler pin is an unusual foreign body in the nasopharynx. Care should be taken while removing these foreign bodies. Digital manipulation for removal of such foreign bodies are hazardous and should be avoided at all cost.

Keywords: Atrophic rhinitis,Ectodermal dysplasia,Nasal myiasis

DOI: 10.5005/jp-journals-10013-1252  |  Open Access |  How to cite  |

Abstract

Aim

To present two rare case of ectodermal dysplasia presenting with maggot infestation due to atrophic rhinitis.

Study design

Case report.

Setting

Tertiary care referral hospital.

Presentation

Two male child aged about 5 and 8 years presented in ENT emergency with history of maggot infestation of nose.

Result

Clinical examination was suggestive of ectodermal dysplasia. Intranasal examination was suggestive of atrophic rhinitis. Maggots were removed in conventional manner. Following conservative treatment, skin biopsy and nasal mucosal biopsy was taken which confirmed the diagnosis of ectodermal dysplasia.

Conclusion

Any ectodermal dysplasia patient should be suspected of having atrophic rhinitis and intranasal conservative therapy should be initiated at the earliest to prevent complication like maggots in nose.
