Complications of Medialization Laryngoplasty (Thyroplasty Type-I)

Abstract

Objective

Medialization laryngoplasty (formally type 1 thyroplasty) is an accepted treatment for unilateral vocal fold paralysis or paresis. The objective of this paper is to ascertain the complications following medialization laryngoplasty in patients with particular reference to implant extrusion.

Study design

The records of 85 patients who underwent medialization laryngoplasty were retrospectively reviewed from January 2001 to July 2009.

Results

There were 3 implant extrusions noted only in female patients with cartilage removal technique.

Conclusions

Implant extrusion rate following medialization laryngoplasty in our institution is comparable to published literature.

Unilateral Vocal Cord Palsy: An Etiopathological Study

Abstract

Objective

The incidence of various causes of unilateral vocal cord palsy (UVCP) has been found to change over time and place. To arrive at the correct diagnosis is important in determining the prognosis as well as the time and mode of intervention. This study intends to evaluate the current etiological profile of unilateral vocal cord palsy in our center and compare it with the previous studies.

Methods

A retrospective study of case records of all consecutive patients with a diagnosis of UVCP presented to Kerala Institute of Medical Sciences, Thiruvananthapuram, Kerala, South India in the period between September 2002 and May 2009 was conducted. The exclusion criteria were all laryngeal and hypopharyngeal malignancies, intubation injuries and cricoarytenoid joint ankylosis. Factors taken for analysis were age, gender, side of palsy and etiology.

Results

A total of 121 cases including 88 males and 33 females in the age range of 2 to 86 years were studied. 61.1% patients had left-sided palsy and 38.8% had right-sided palsy. The incidence of various etiologies were idiopathic (42.1%), surgical trauma (22.3%), nonsurgical trauma (6.61%), nonlaryngeal malignancy (6.61%), central (12.4%) and other benign lesions (9.09%). The incidence of all nonthyroidectomy surgeries together (59.3%) was more than that of thyroidectomy (40.7%). The most common individual surgical procedure was still thyroidectomy (11 cases, 40.7%) followed by coronary artery bypass grafting (CABG) (7 cases, 25.9%).

Conclusion

Idiopathic vocal cord palsy constituted the major subgroup. Thyroidectomy continues to be the single most common surgical procedure responsible for vocal cord palsy. Cardiac surgeries, trauma and cerebrovascular accidents are also increasingly causing vocal cord palsy, which is suggestive of the changing trend in life style and life expectancy. The right recurrent laryngeal nerve is not at higher risk than the left in thyroid surgery. Benign thyroid swellings also contribute significantly to UVCP.

Abstract

Laryngeal cancers form a major bulk of head and neck malignancies. If diagnosed early, the aggressive management can offer good cure rates and prognosis. Endoscopic excision is a viable alternative to radiation therapy along with its predictable consequences and complications. The author describes the usage of classical phonosurgery techniques in endoscopic extirpation of early laryngeal cancers.

Abstract

Puberphonia is a psychogenic voice disorder; so far the treatment for this one is speech therapy. Even though there is no organic lesion, surgical treatment holds good for the resistant cases who does not improve with speech therapy. Here, we are sharing our experience of 7 cases with proper preoperative and postoperative voice analysis.

Hoarseness of Voice: A Retrospective Study of 251 Cases

Abstract

Aims

Hoarseness of voice is a common symptom in otolaryngological practice and it is the earliest manifestation of a large variety of conditions directly or indirectly affecting the larynx, ranging from benign to most malignant. This study was undertaken to find out clinical profile, predisposing factors and etiology of hoarseness of voice.

Methods

A retrospective cohort analysis was carried out in Department of ENT, Pt JNM Medical College, Raipur, in 251 cases of change in voice for 3 years duration. All cases were analyzed for detailed history and underwent pre- and postoperatively stroboscopic examination to reach the diagnosis.

Results

Total 251 cases with M:F ratio of 1.9:1 were analyzed. Patients age ranged from 11 to 80 years and majority of patients equally presented in 4th and 6th decade. Nonvocal/nonprofessional group constituted as a single largest group (85.26%). Smoking was commonest predisposing factor (44.22%) followed by vocal abuse (30.28%). Out of 251 cases, 83.67% cases were organic and 16.33% cases were functional in origin.

Correlation between Rigid Laryngoscopy and Histopathology of Laryngeal Lesions at Our Voice Clinic

Abstract

Introduction

In today's world of rapid technological advances, the medical field is developing at a rapid speed. Various sophisticated tools are available for the diagnostic work-up of patients with voice disorders. These are stroboscopy, optical coherence tomography (OCT), contact endoscopy and laryngeal USG, which gives sophisticated details of the larynx. However, at present most of these modalities are expensive and available only at few research centers. Rigid laryngoscopy is a noninvasive, easily available and fairly accurate diagnostic tool in patients with voice disorders.

Aims and objectives

The aim of our study was to assess the diagnostic potential of rigid laryngoscopy in different laryngeal lesions and its correlation with histopathology. This is a one year retrospective study at our voice clinic at Bombay Hospital, Mumbai.

Patients and methods

We examined 720 patients at our voice clinic from January 2008 to December 2008. Microlaryngeal surgery was performed on 59 of these. The clinical diagnosis was made after detailed history taking and clinical examination by a 70 degree Hopkins rod telescope.

Results

In our study, clinical diagnosis was 100% accurate in vocal fold subepithelial cyst, vocal fold nodules, laryngeal papilloma and contact granuloma. The clinical diagnosis had a reliability of 30% for Vocal fold polyps, 50% for leukoplakia, and 66% for malignant lesion. In suspected laryngeal polyps, leukoplakia and malignancy, our study indicates that we need to excise the lesion surgically and prove the histopathology.

Conclusion

Every tissue removed during laryngeal surgery should be sent for histopathology. Rigid Laryngoscopy is a safe and accurate tool to diagnose nodules and cysts.

Abstract

Neurogenic tumors of the larynx are extremely rare. Here, we are reporting a case of laryngeal schwannoma presenting as supraglottic submucosal lesion with hoarseness and stridor, which was excised by lateral pharyngotomy approach. This presentation is to further discuss the histology of neurogenic tumors, modes of presentation, differential diagnosis and treatment modalities.
