Influence of the Grander Technology in the Physical Properties of the Self-Etch Adhesive System

Abstract

Aim

The objective of this study was to evaluate the influence of the Grander technology in reducing the surface tension and contact angle of self-etch adhesive system.

Materials and methods

Distilled water and Xeno III self-etch adhesive system (Dentsply) were modified by physical contact with the flexible unit Grander system to revitalize water, for 48 hours, resulting in four groups: Group CW-distilled water under normal conditions; Group CA-Xeno III adhesive system under normal conditions; Group GW-distilled water system modified by the Grander system; Group GA-Xeno III adhesive system modified by the Grander system. Surface tension and contact angle of the adhesive system and water in normal and Grandermodified conditions was measured with a goniometer. ANOVA and Tukey test were used to analyze the results (5%).

Results and conclusion

For surface tension, ANOVA showed p < 0.05, what indicated significant differences between the groups. The mean values (D/cm) for the groups were: CW-72.4 a; GW-69.45 b; GA: 31.17 c; CA-29.98 c. The results showed a significant reduction of surface tension for distilled water modified by Grander system. Grander technology did not interfere in the surface tension and contact angle physical properties of the Xeno III self-etch adhesive system.

Clinical significance

Grander technology is a method that revitalizes water by physical spatial restructuring of its molecules. The modify of an adhesive system, also a liquid with an aqueous solvent, can provide a reduction in surface tension and contact angle, increasing its wetness capacity and therefore ensuring a greater diffusibility.

Effectiveness of Antimicrobial Solutions on Streptococcus mutans in used Toothbrushes

Abstract

Purpose

The objective was to determine the effectiveness of antimicrobial solutions on Streptococcus mutans in used tooth brushes.

Methods

Sixty children used their toothbrushes twice a day, for seven consecutive days. The toothbrush bristles were then immersed into antimicrobial solutions for 12 hours: Group I-3% neem; group II-5% turmeric; group III-0.5% cetylpyridinium chloride, group IV-0.2% chlorhexidine gluconate. They were then placed into test tubes containing the resultant suspensions were three-fold diluted. Aliquots of 0.1 ml were plated in Mitis Salivarius Agar (enriched with sucrose) using dilution and plating method. Incubation was done in an anaerobic jar for 72 hours at 37°C.

Results

The results obtained showed that there was 88% reduction in the Streptococcus mutans in the 3% neem group, 86% reduction in the Streptococcus mutans in the 0.5% cetylpyridinium chloride group,78% reduction in the Streptococcus mutans in the 5% turmeric group, 65% reduction in the Streptococcus mutans in the 0.2% chlorhexidine gluconate group. The difference between them was found to be statistically significant (p < 0.001).

Conclusion

Bacterial contamination of toothbrushes was a major cause of concern. All the antimicrobial solutions used in this study proved to be effective toothbrush decontaminants. The efficacy of 3% neem was highest in our study followed by 0.5% cetylpyridinium chloride, 5% turmeric and 0.2% chlorhexidine gluconate.

Patient Attendance for Emergency Care in a Brazilian Dental School

Abstract

Objective

The objective of this study was to analyze the profile of patients attending the Emergency Dental Clinic at São José dos Campos Dental School, UNESP-São Paulo State University, between 2006 and 2010, evaluating the gender, ethnicity, age, diagnosis and treatment performed for resolution of the cases.

Materials and methods

A total of 600 dental records were analyzed by descriptive statistics, Fisher and Chi-square tests at a significance level of 5%. The dental records included information on the patients’ name, gender, age, ethnicity, phone number, date of attendance, diagnosis and treatment performed. The case history addressed the chief complaint, medical and dental history of the patient. The clinical evaluation comprised tests, such as visual examination of the intraoral hard and soft tissues, tooth percussion, palpation, probing, pulp sensitivity test and radiographic examination. The diagnosis and treatment for each case were established based on the case history and clinical tests.

Results

There was predominance of female gender (59.17%) and patients aged 20 to 39 years (49.67%). Most procedures were performed to solve endodontic emergencies (37.5%), followed by placement of dressings (10.6%), tooth extraction (8.9%), periodontal problems (4.6%) and cementation of provisional crowns or definitive dentures (4.5%).

Conclusion

Dental caries and its consequences were the main factors involved in the emergency attendances. There is a constant need of health promotion, considering that the patients searched for restorative treatments without the necessary emphasis on prevention. Additional studies are needed to determine strategies for decreasing the use of emergency services for nonemergency dental problems.

Clinical relevance

The diagnosis and treatment of dental emergencies are challenging. If inadequately performed, they may cause difficulties or even failures in pain relief, worsening or delaying the continuation of treatment planning and accomplishment.

Evaluation of Salivary Antioxidant Enzymes among Smokers and Nonsmokers

Abstract

Background

Cigarette smoke contains various oxygen-free radicals which are considered as the main causes of damage to biomolecules when exposed to cigarette smoke. Saliva is the first biological fluid that encounters inhaled cigarette smoke (CS) and has an antioxidant defense system able to counter toxic activities of free radical species. So, the aim of this study was to compare the levels of superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px) in saliva of smokers and nonsmokers.

Materials and methods

Unstimulated saliva of 200 males (100 smokers and 100 nonsmokers) was collected. The saliva was centrifuged and the activity of salivary superoxide dismutase and glutathione peroxidase was measured according to a specific assay.

Results

The mean value of superoxide dismutase activity was significantly higher in the smoking group than in the nonsmoker group, while the levels of GSH-Px activity was significantly higher in the nonsmoking group than in the smoking group.

Conclusion

Cigarette smoke leads to an alteration in salivary antioxidant activity. Evaluating the variations in the level of antioxidant enzymes (SOD and GSH-Px) in smoker's saliva might be useful for estimating the level of oxidative stress caused by cigarette smoke. Thus, it may help in patient's education regarding the ill-effects of smoking and determining the evolution and progress of various oral diseases.

Oral Health Status, Hygiene Habits and Treatment Needs among Elderly Brazilians: A Cross-sectional Study

Abstract

Purpose

Oral health status seems to play an important role in the quality of life of the elderly, the aim of this study was to verify oral hygiene habits, oral health status and treatment needs of a sample of elderly Brazilians in the city of São Paulo.

Materials and methods

A convenience sample of 167 individuals underwent interviews and clinical examinations.

Results

A total of 107 patients (64.1%) reported brushing their teeth 3 or more times per day, 42.5% floss their teeth, and 24.0% uses mouthwash. The average time since the patients’ last visit to a dentist was 2.2 years (±4.6). Only 24.6% of the teeth were periodontally healthy, and the most prevalent pathological condition was gingival recession (46.1%). Eighty-two percent use some type of dental prosthesis. A total of 35.3% of the sample required periodontal treatment, 32.9% needed direct restorations, 39.5% removable partial dentures and 23.4% complete dentures. Positive associations were found between the current condition of oral health and flossing (p = 0.004) and between the current condition and the use of interdental brushing (p = 0.009), and an inverse association with the time elapsed since the last dental appointment (p = 0.016).

Conclusion

The oral health status in this sample was better than observed in previous Brazilian surveys, but dental treatment needs remain high, mainly regarding prosthetic needs.

Abstract

Objective

To evaluate marginal discrepancies between cast implant superstructure and EDM processed implant structure.

Materials and methods

For the purpose of this study, an experimental model was prepared on which nine cast implant superstructure in nickel-chromium were fabricated which was then subjected to EDM processing. One-screw test was used to verify fit of superstructure both pre- and post-EDM processing. Photo/Image analyzer was used to evaluate the marginal discrepancy. To reduce the error of measurement, nine readings were taken of each implant abutment junction, i.e. overall 324 readings pre- and post-EDM processing was made.

Results

All implant superstructure fabricated by investment casting procedure in nickel-chromium alloy showed negative one-screw test, i.e. they were fitting actively on abutment head and were sitting passively after EDM processing. Mean marginal opening of cast implant superstructure before EDM processing was 95.93 microns and 22.76 microns after EDM processing.

Conclusion

Nickel-chromium alloy can be used for fabrication of implant superstructure with EDM as programmed refining procedure to achieve passive fit.

Abstract

Introduction

The increasing use of pan masala/gutkha, a mix of tobacco and a less moist form of betel quid lacking the betel leaf, is associated with an earlier age of onset of oral submucous fibrosis. Hence, current study was conducted to evaluate and compare clinical, histopathological and cytological features of OSMF due to betel quid (BQ) and pan masala/gutkha chewing (PMG).

Materials and methods

Patients who had habit of chewing BQ or PMG and who were suffering from OSMF were selected for the study. Buccal mucosal smears were obtained from 30 controls, BQ and PMG chewers. Incisional biopsy was done on buccal mucosa of BQ and PMG chewers. The results were tabulated and statistically analyzed.

Results and observation

PMG chewing is associated with onset of OSMF at an early age than BQ chewing habit. Relatively large proportion of intermediate cells was present as compared to superficial cells of buccal mucosa in habitual chewers as compared to control. However, intermediate cells capable of proliferation were slightly higher in PMG chewers as compared to BQ chewers. On histopathological examination connective tissue hyalinization is more frequently evident amongst BQ chewers than PMG chewers.

Conclusion

Habitual chewing of PMG induces OSMF more rapidly than the traditional BQ.

Abstract

Aims

To determine the effect of self-etchant pH on shear bond strength of orthodontic brackets; to compare the shear bond strengths of brackets bonded with three SEPs and brackets bonded with conventional etch, rinse, bond method and to find the brackets/adhesive failure mode.

Materials and methods

One hundred and twenty premolar teeth were cleaned, mounted, and randomly divided into four groups of 30 samples each- Transbond XT conventional etch and bond system (control), Adper SE Plus SEP (3M ESPE) with a pH of 0.9 to 1.0, Transbond Plus SEP (3M Unitek) with a pH of about 1.0 and Clearfil SE Bond SEP (Kuraray America) with a pH of around 2.0. All teeth were bonded with Transbond XT paste (3M Unitek). The teeth were debonded within half an hour after initial bonding by using a universal testing machine. The residual adhesive on each tooth was evaluated. ANOVA was used to compare the shear bond strength (SBS) of the three groups, and the Chi-square test was used to compare the adhesive remnant index (ARI) scores for the three groups.

Results

ANOVA indicated significant differences between the groups. Clearfil attained the SBS (6.5 ± 0.6689 MPa) closest to the control group, whereas Adper inspite of being the most aggressive recorded the lowest SBS (5.7 ± 0.5695 MPa). Transbond self-etching primer achieved a mean SBS of 6.1 ± 0.6211 MPa. However, all the three SEPs recorded SBS which was significantly less than that of Transbond conventional etch, rinse and bond system (11.8027 ± 0.8059 MPa). The comparisons of the ARI scores between the three groups indicated that bracket failure mode was significantly different between the three groups (p < 0.05).

Conclusion

These findings show that factors other than pH, such as the ability of the bonding adhesive to form a chemical bond to enamel and the strength of the bonding adhesive itself, significantly influence the SBS of orthodontic brackets.

Abstract

Method

PubMed and relevant professional guidelines were searched for research and for evidence-based guidelines.

Outcome

There is a need for evidence-based guidelines developed by stakeholders including the American Academy of Orthopedic Surgeons, the American Dental Association and the Infectious Disease Society. There is an absence of level 1 evidence for or against the use of prophylactic antibiotics in patients with prosthetic joints undergoing invasive dental treatment. Therefore, until the professional organizations provide evidence-based guidance, professional judgment must depend on the client history following joint replacement and the state of the host immune response.

Abstract

Background

Considering the limited qualitative and quantitative bone in the posterior arch, this modality of prosthetic treatment could provide a positive emotional factor reestablished by immovability of the anterior fixed implant-supported segment.

Objective

This clinical report demonstrates the possibility of achieving positive results with a removable partial denture connected to an implant-supported fixed prosthesis associated to an extra resilient attachment.

Clinical significance

In cases of posterior mandibular and maxilla atrophy added to the patients desire against the bone graft, this kind of prosthetic treatment has an important place as an alternative.
