Dental Caries and Oral Health-related Quality of Life in Cleft Lip and Palate Patients: A Pilot Study

Abstract

Background

Dental caries is a chronic disease caused by a set of biological and social factors. The aim of this study was to conduct a study of dental caries and oral health related quality of life (QoL) in Brazilian patients with cleft lip and palate.

Materials and methods

A cross-sectional study was conducted with cleft lip and palate patients with 12 years of age. Participants were examined for dental caries decayed, missing and filled teeth-World Health Organization (DMFT-WHO criteria). The dental impact on daily living (DIDL) instrument was used to collect data of oral health-related QoL. The Mann- Whitney test was used to present the differences between gender and the Spearman correlation was used with DIDL and DMFT.

Results

The 30 (30%) of the 50 children with cleft lip and palate were caries-free, and the DMFT was 1.70 (1.62), with 53.91% for the care index. The dimensions 'speak’ (34%) and ‘Hygiene’ (46%) represented the largest negative impact on QoL. The DIDL instrument correlated only among the missing component and dimensions 'stress’ (r = 0.35, p < 0.01) and 'study’ (r = 0.52, p < 0.01).

Conclusion

The presence of cleft lip and palate in subjects 12 years of age had a negative impact on QoL and this impact was also significant for tooth loss caused by dental caries.

Abstract

Introduction

This study has used cone-beam computed tomography (CBCT) to evaluate the efficacy of two different nickel titanium (NiTi) rotary retreatment systems for removal of filling material from teeth obturated with contemporary endodontic sealers. The operative time required for removal of root filling materials was also evaluated.

Methods

Sixty root canals were randomly divided into three groups according to the sealer used for obturation: AH Plus Jet, mineral trioxide aggregate (MTA) Fillapex, and RoekoSeal Automix (RSA) (all, n = 20). After obturation, each group was subdivided into two subgroups according to the NiTi retreatment rotary system: Mtwo retreatment (Mtwo-R) file subgroup and ProTaper-UR subgroup—both instrumented with Mtwo-R files. Teeth were scanned with a microcomputed tomography before and after obturation.

Results

Residual filling material was observed in 100% of the specimens after retreatment. No difference was found between Mtwo-R and ProTaper-UR systems (p > 0.05). The RSA was more effectively removed from dentin walls than AH Plus Jet (p < 0.05), with no difference to MTA Fillapex (p > 0.05). Thus, teeth obturated with RSA required less operative time to be retreated (both, p<0.05).

Conclusion

ProTaper-UR and Mtwo-R systems were effective in removing the filling materials after instrumentation, but they could not eliminate them completely. Moreover, teeth obturated with silicone-based sealer RSA showed less residual filling material in dentin walls compared to the epoxy resin-based sealer (AH Plus Jet), which exhibited no difference compared to the salicylate resin-based sealer (MTA Fillapex). Teeth obturated with silicon-based sealer also required less operative time to be retreated despite the NiTi rotary retreatment system tested.

Abstract

Background

To describe oral lesions diagnosed during oral cancer prevention campaign performed in Fernandópolis, Brazil, in 2015.

Materials and methods

Patients who attended for the Basic Health Units of the city of Fernandópolis on the day of the campaign were examined by dental surgeons who searched for oral lesions clinically suggestive for oral squamous cell carcinoma (SCC) or potentially malignant lesions. Final diagnosis was obtained by a re-evaluation of patients with suspicious lesions performed by an expert in oral diseases.

Results

Seven hundred and sixty-two patients were examined during the campaign; although 96 (12.59%) presented with oral suspicious lesions and forwarded for re-evaluation, only 72 (9.44%) attended for re-evaluation and got the final diagnosis. Among re-evaluated patients, only 1 (1.38%) was diagnosed with oral SCC, whereas 56 (77.77%) were diagnosed with oral benign lesions, and 19 (26.38%) were diagnosed with oral normality variations.

Conclusion

Oral cancer low diagnosis rate detected during this campaign might be attributed to lacks on oral cancer prevention campaign methodologies, which clearly needs to be improved aiming to reach patients in real risk for oral cancer development. Furthermore, oral benign lesions diagnosis among patients forwarded with suspicious lesions rates suggests a lack on dental surgeons’ knowledge regarding oral cancer.

Prevalence of Molar Incisor Hypomineralization and its Relation with Dental Caries in School Children of Udupi District, South India

Abstract

Aim

To evaluate the prevalence of molar incisor hypomineralization (MIH) and its relationship with dental caries in school children of Udupi district.

Materials and methods

A total of 352 school children were recruited for this study. The European Academy of Pediatric Dentistry (EAPD) 2003, criteria were followed for diagnosis of MIH. Dental caries was recorded using World Health Organization (WHO) guidelines (1997) under natural day light.

Results

The prevalence of MIH was seen to be 27% (n = 95). A total of 323 teeth had hypomineralization (106 were incisors and 217 were molars). The lower molars (n = 127) were more frequently affected than the upper molars (n = 90). Overall the maxillary incisors (n = 68) were more affected than the mandibular incisors (n = 38). Amongst the children with MIH, the mean number of decayed teeth was significantly lower than mean number of hypomineralized teeth in first permanent molars and first permanent molars and incisors (p < 0.001 and < 0.001) respectively. The mean decayed teeth in first permanent molar and incisors were significantly higher in subjects with MIH than without MIH (p < 0.001). Poisson regression analysis showed a significant positive association between MIH and dental caries (B = 1.43; p < 0.001).

Conclusion

The prevalence of MIH was relatively high and was associated with dental caries.

Abstract

Objective

To investigate the variation of the soft palate morphology and the proportional differences of the morphology of soft palate between males and females.

Materials and methods

We conducted a cross-sectional study in department of oral medicine and radiology. Lateral cephalometric radiographs of individuals subjected to radiographic examination for orthodontic purpose were retrieved. A total of 100 digital lateral cephalograms of normal healthy individuals (50 males and 50 females) who were aged 15 to 45 years were retrieved.

Results

Type 1 was most commonest shape of the soft palate (30%) followed by type 6 (19%), types 2 and 3 (17 and 17%), type 4 (11%) with least being type 5 (6%). There was no significant difference in the distribution of shape of soft palate between males and females (p = 0.312).

Conclusion

This classification can help us better understand the diversity of the velar morphology and can be used as references for the research of velopharyngeal closure in cleft palate individuals, obstructive sleep apnea syndrome (OSAS) and other conditions.

Comparison of the Pain Perception of Conventional and Computer-controlled Anesthesia Techniques for Extraction of Maxillary First Premolar

Abstract

Introduction

Anterior middle superior alveolar (AMSA) block is a nerve block technique in conjunction with computer-controlled local anesthetic delivery (CCLAD) system. Anterior middle superior alveolar produces local anesthesia (LA) to the maxillary teeth from second premolar to the central incisor including the hard palate and corresponding soft-tissue of both buccal and palatal side, with single palatal injection.

Aim

To evaluate efficacy of AMSA block for extraction of maxillary first premolar.

Study design

In randomized single blind, split mouth clinical trial, the 14 patients received the conventional buccal-palatal (B-P) anesthesia on one side and CCLAD anesthesia AMSA for other side. The treatment consisted of bilateral extraction of maxillary first premolars for orthodontics. Pain perception ratings were obtained by using 10 point visual analog scale (VAS).

Results

A total of 100% cases had painless injection with CCLAD, whereas 28.57% patients had painless injection experience with conventional LA. In all cases, there was no pain during extraction on both sides. With CCLAD, buccal retraction was painful in four cases whereas palatal retraction was painful in one case.

Conclusion

Anterior middle superior alveolar block injected with CCLAD is better as LA technique for extraction of maxillary first premolar as compared to B-P approach.

Assessment of Parent's Preference to General or Local Anesthesia for Children undergoing Dental Treatment

Abstract

Background

Investigators have begun to address the relationship of parental satisfaction with dental care for children since the dentist/physician and the patient are all aware of the services being provided. These determining characters are essential for compliance of the patient, as patient satisfaction is directly related to health-related issues, needs, and practice. Patient satisfaction has an important role in determining the utilization of the healthcare services and the compliance behavior of the patients. Factors which can influence the patients’ decision to seek care and follow through to the completion of the treatment process can affect the physiologic and functional outcomes of the treatment. Hence, this survey was done to evaluate parent's preference to general or local anesthesia in their children's dental treatments.

Materials and methods

In this survey, a modified and adapted questionnaire from a previously tested survey that was used to assess parental satisfaction with dental treatments under general anesthesia in pediatric dentistry, was used to assess parents’ preference to general or local anesthesia. Parents whose children were going to undergo treatments in Department of Pedodontics, Saveetha Dental College, was issued with this questionnaire and clinical examination of teeth present and treatment to be done was examined.

Results

Ninety-seven percent of the parents preferred local anesthesia to general anesthesia. Parents preferred general anesthesia most only in case child below the age of 3 and when multiple settings are required of and their knowledge about anesthesia and its side effects was found to low and preferred local anesthesia in most cases.

Conclusion

Parents play a unique role in dental care and over all well being of the child. Some parents and dental practitioners prefer the presence of a parent with their child during the delivery of in-office dental care. Parents are aware of their child's previous dental experiences and are also aware of the child's interest and fear. Most of the parent's preference was to local anesthesia (97%) and preferred general anesthesia under given circumstances.

Gonial Angle as an Indicator for the Prediction of Growth Pattern

Abstract

Aim

To determine the gonial angle, upper gonial angle and lower gonial angle in patients with horizontal, vertical and average growth pattern belonging to the local Chennai population and determine if it can be used as a growth indicator.

Materials and methods

Gonial angle, upper gonial angle and lower gonial angle measurements were made on lateral cephalograms of 90 patients—44 males and 46 females.

Results

A one sample t-test was used to determine the mean values for average, horizontal and vertical growth pattern and to check their level of significance. All the values were statistically significant with a p-value = 0.000.

Conclusion

The upper gonial angle was the same irrespective of growth pattern. The gonial angle and lower gonial angle can be used as an indicator for growth.

Assessment of Learning Preferences among Indian Undergraduate Dental Students

Abstract

Objectives

In order to facilitate and optimize learning process, there is need to evaluate preferred choice of teaching methods among dental students, especially in Indian scenario wherein lecturing remains the most commonly employed method of teaching. The aim of the present study was to assess and compare learning approaches among the male and female undergraduate dental students.

Materials and methods

The study was conducted among undergraduate dental students of Manipal College of Dental Sciences, Manipal University, Mangalore, Karnataka, India. Students belonging to the first 4 years of their professional dental education program were enrolled in the study. A structured, self-administered questionnaire comprising of nine multiple choice questions apart from the demographic details was employed to assess the dental student's preferences towards learning.

Results

A total of 100 dental students participated in the study. Majority of the respondents preferred lecture classes in the morning hours for 30 minutes. Audiovisual method was the most preferred teaching aid, whereas multiple choice questions and periodic tests were most preferred method for evaluation. Significant difference was observed between male and female study subjects in terms of average percentage of the lecture content they were able to understand (p<0.035).

Conclusions

The present study holds policy implications for changes in the teaching patterns practiced in current times. Differences among learning preferences of males and females must be considered while selecting teaching methods. There is need to introduce more of active learning programs in order to overcome the drawback of lack of interaction and also to facilitate the learning among all students.

Abstract

Background

To evaluate microhardness of enamel surface after microabrasion of enamel using casein phosphopeptideamorphous calcium phosphate (CPP-ACP) and casein phosphopeptide-amorphous calcium phosphate with fluoride (CPP-ACPF).

Materials and methods

Ten freshly extracted anterior teeth were selected. Teeth were treated with abrasive slurry. This prepared slurry was applied to the labial aspects of teeth, using a rotating rubber cup, for 50 seconds. Each tooth was divided into four parts and treated accordingly with the topical application of the demineralizing agent stored in artificial saliva.

The teeth were divided into four groups, as follows:

– Group A: Control group

– Group B: Microabrasion done but not treated

– Group C: Microabrasion done and CPP-ACP paste applied

– Group D: Microabrasion done and CPP-ACPF (i.e. with fluoride) applied.

The microhardness was evaluated using the Vicker's microhardness test.

Results

Microhardness values were highest for the control group followed by group D, group C and finally group B. Microabraded group of teeth with no topical application showed least microhardness values of all.

Conclusion

The CPP-ACPF increased the microabrasion significantly as compared to CPP-ACP and microabrasion group.
