We present a case of the patient with fixed dilated pupil after deep anterior lamellar keratoplasty (DALK). 6 months after DALK transplant was clear as well as the natural lens. Patient complaints included low vision and severe photophobia. Surgical reconstruction of the pupil was performed through corneal paracentesis after filling the anterior chamber with cohesive OVD.
