This 73-year-old patient is four years status post-bilateral diffractive multifocal IOL implantation. She is unhappy with her uncor­rected vision in each eye, but even with spectacles, she is complaining about the visual quality in one eye more than the other. She has posterior synechiae to the capsulorhexis, and has had a YAG capsulotomy. Because of higher order aberrations in the more symptomatic eye that seem to suggest IOL decentration or tilt, the multifocal IOL is exchanged for a monofocal
