This video demonstrates the technique of the posterior assisted levitation (PAL) technique for a descending heminucleus following posterior capsular rupture during phaco. Among the principles shown are the support and levitation of the nucleus with the PAL technique, the “Viscoat Trap” method of supporting residual lens material during vitrectomy, and the bimanual pars plana anterior vitrectomy approach. Principles for converting to a larger incision manual ECCE are also demonstrated.
