This video demonstrates the technical aspects of anterior transposition of the facial nerve in lateral skullbase surgery. After performing a subtotal petrosectomy, the mastoid and tympanic portions of the facial nerve are deroofed. This is followed by sharp dissection on the region of the stylomastoid foramen to identify the facial nerve. A bipolar cautery may be used to cauterize the branches of the stylomastoid artery. Once the nerve has been traced in its entire course, it may be transposed anteriorly.
