This is the preferred technique by the authors for the excision of vestibular schwannomas. It involves drilling 270 degrees around the internal auditory meatus, which may be extended further depending upon the transapical extension of the tumor. The most important specific preoperative investigation when using the translabyrinthine approach is a pure tone audiogram. A vestibular schwannoma with unserviceable hearing is an indication for this approach. Additionally, size of the tumor is very important while using this approach.
