This video demonstrates the expanded endoscopic approach for the excision of a sellar and suprasellar meningioma. While the expanded approach to the skullbase using the operating microscope is not new, its applications to lesions beyond the sellar and suprasellar region remain limited. The increasing use of the endoscope with its improved illumination and wider field of view along with current sophisticated neuronavigation systems have shown the feasibility, safety and potential advantages of the approach for selective midline lesions of the skullbase well beyond the sellar region. In this context, and based on the authors’ experience, with large number of purely endoscopically performed pituitary surgery, the expanded endoscopic endonasal approach has evolved in their clinic and has enabled them to access and remove lesions involving the ventral skullbase from the frontal sinus to the lower clivus and foramen magnum. This video illustrates the use of the expanded endoscopic endonasal approach for the removal of meningioma.
