This video demonstrate phaco on soft cataracts. With soft nuclei, it is often difficult to catch and lift up, break and emulsify the nucleus. It is advisable to change from burst to pulse mode and apply minimum ultrasound. Most of the nucleus and epinucleus can be removed in foot pedal position 2, which is for irrigation/aspiration. Modern phaco machines have a very fast rise of vacuum and, if not handled carefully, the posterior and anterior capsule and the iris could be absorbed easily into the phaco tip and damaged. Therefore, aspirating cortex with the phaco tip should be done only by experienced surgeons. If the nucleus is not hard enough to catch, the surgeon can remove this nucleus and cortex just with irrigation/aspiration and without any ultrasound. It is important to work in a systematic way when breaking and emulsifying the nucleus. Try to break the nucleus first into two halves and then break smaller pieces and emulsify them and always turn the nucleus into the right position towards the phaco tip.
