This video highlights minimally invasive transforaminal lumbar interbody fusion. The retractor should be centered over the facet joint with a parallel and medial trajectory to the disc space. The remaining muscle and soft tissue is removed with electrocautery and rongeurs for adequate visualization. A high-speed burr is utilized to remove the lamina and the inferior facet of the cephalad vertebrae. The retractor should be centered over the facet joint with a parallel and medial trajectory to the disc space. Inadequate resection of the facet joint will reduce the visualization of the disc space. This may result in the placement of an undersized cage, thus increasing the likelihood of cage migration and pseudarthrosis. It is important to resect the entire superior articular process at the L5-S1 level to lateralize the tubular dilator starting point while medializing its trajectory.
