This video demonstrates drilling of the bone tunnels during UCL reconstruction. The ulnar bone tunnel is drilled first at the apex of the sublime tubercle. Small bone curettes are used to enlarge the tunnel and ensure no graft tunnel mismatch occurs. Bulb irrigation is used to ensure patency and connection between all tunnels. The humeral tunnels are drilled with the lateral limb first, followed by the medial short limb and connected in the same fashion as the ulnar tunnel. The ulnar nerve is protected throughout the procedure.
