This is a recording from a patient with mitral stenosis. It is taken from the 2nd left interspace. Is there an OS audible in this recording taken at the 2nd left interspace? Yes, one can hear an OS rhythm. A fairly loud S1 followed by split S2, which could be A2-OS. One can hear the examiner’s voice instructing the patient to breathe in and out. On inspiration, following the inspiratory noise, a typical trill is audible. The triple sound is quite characteristic and confirmatory of the presence of the OS.
