This recording is from a patient with a cardiomyopathy taken from the apex area. There is a tachycardia with increased heart rate. The S1 and S2 are replaced by a systolic murmur followed by a diastolic sound. The tachycardia provides a gallop rhythm. Intermittently one can actually hear two sounds in diastole one the S3 and the other the S4, which this patient also had. The S4 is well separated from the murmur due to a relatively long PR interval also. The rhythm is somewhat like the following: shooooooooo…..Bum….Bum.. shooooooooo…..Bum….Bum..
