This recording is from a 17 year old patient with a long PR interval of 0.28 sec. This patient being young would have reason to have a physiologic S3. In addition, the atrial contractions tended to occur during the rapid filling phase of diastole due to the long PR interval. This would tend to augment the early diastolic inflow velocity providing more energy to the moving column of blood. Therefore the S3 would be expected to become exaggerated. This is termed summation gallop.
