This is a recording from a patient with a systolic murmur and atrial fibrillation. The rhythm is irregular with changing cycle lengths. Could you guess the nature of the lesion causing the systolic murmur? The intensity of the murmur does not change very much following the long diastolic intervals suggesting that this is a regurgitant murmur most likely mitral regurgitation since the recording is from the apex and presumably maximally loud there. The harshness of the murmur and the mixed frequencies suggest the regurgitation to be severe with a lot of flow.
