This recording is from a patient with mitral stenosis and atrial fibrillation taken from the apex area. What is the quality of the S1? Is an OS heard? What is the severity of the mitral stenosis? The rhythm appears to be irregular. The S1 is quite loud. There is no clear-cut OS audible in this recording. The murmur appears to be diastolic and long and ends with the loud S1. The murmur appears to be a low frequency rumble, which lasts throughout diastole despite the varying lengths of the diastoles due presumably to atrial fibrillation. Full length diastolic rumble even in long diastole suggests a full length gradient across the mitral valve, which will make the stenosis significant.
