The audio recording is from a 45 year old woman from India who is being evaluated for mild exertional dyspnea. The recording is from the left sternal border area. All of the following statements are correct except: (A) The S1 is loud and the S2 is well split, (B) The diastolic murmur is suggestive of aortic regurgitation, (C) The diastolic murmur is suggestive of mitral stenosis, (D) The diagnosis is rheumatic heart disease with probable mitral stenosis and aortic regurgitation. The diastolic murmur is high frequency and decrescendo as in aortic regurgitation. The S1 is loud with a split S2 effect due to an OS. This patient in fact had rheumatic heart disease with mitral stenosis and aortic regurgitation. However, the recording is from the left sternal border area, where only the aortic regurgitant murmur is heard.
