The video describes transoral robotic lingual tonsillectomy/base of tongue resection for obstructive sleep apnea. This video visualization of the entire glottic larynx anatomy and retraction is performed at a left posterior angle. For adequate BOT tissue excision, the monopolar instrument or laser if used should be angled superiorly. Dissection should not disrupt the palatine tonsil pillars and the surgical assistant may help in retracting the pillars and epiglottis away from the surgical site. Once a complete history and physical examination have been performed, a PSG obtained and the degree of multilevel obstruction has been established, the patient will be consented for flexible fiberoptic sleep nasendoscopy and other indicated surgical procedures such as septoplasty, turbinate reduction, tonsillectomy and UPPP. Surgical planning includes first having a sedated fiberoptic sleep endoscopy.
