The video demonstrates the transoral robotic assisted free flap inset in the oropharynx. For flap inset, two needle drivers are placed on either arm. Initially, the flap is tacked with silk suture. This allows for the surgeon to assess the size match, establish the orientation, assess vessel length and prevent kinking or twisting of the vessel. If a pedicled flap is being used, the surgeon may proceed with insetting of the flap. If a free flap is being used, after tacking the flap, the microvascular anastomosis can begin.
