The patient presented 6 days after anterior wall myocardial infarction with shortness of breath: Apical 4-chamber view showing a rent in the apical ventricular septum suggestive of ventricular septal rupture. There is also a large serpentine clot floating in the right atrium. This is likely an embolized clot from deep vein thrombosis in the legs.
