This video clip showing parasternal long-axis view in a patient with posterior wall infarct. Note the wall motion defect in mid-posterior wall along with flail A2 scallop of the anterior mitral leaflet. The fragment of the ruptured papillary muscle head is seen attached to the tip of the anterior mitral leaflet and is freely moving in and out of the left atrium.
