This video clip zoomed apical 4-chamber view in a patient with anteroapical infarction showing a large apical pseudoaneurysm with a shelf-like structure at its origin. Note the relatively narrow neck of opening and large aneurysmal cavity, which is partially layered with clot. Spontaneous echo contrast is seen in the aneurysm as swirling smoke.
