In starting donor harvesting, establishing the initial drill rotational speeds (RPM) on motorized or mechanically assisted extraction devices to the slowest effective speed is recommended. Since the punch is hollow, resistance on the punch increases as the follicle moves toward the inside portion of the punch. Typically what is detected audibly is a slight decrease in RPM upon skin penetration by the punch. Simultaneous upward retraction or tension on the skin before advancing the punch increases the hair exit angle, so the angulation of the punch prior to scoring the dermis is easier for the physician operator. Tension on the skin and tumescence of the tissue also help to minimize the follicle displacement and temporarily reduce the skin elasticity that in turn decreases follicular transection. The physician operator may be required to increase the RPM of the punch if necessary for more successful donor harvesting of the grafts.
