This video presents recipient-site creation: crown/vertex design. Like the temple, recipient-site creation for the crown/vertex remains a highly advanced subject. Creating precise vertex recipient sites requires technical precision, patience, and artistry. Before creating recipient sites for the vertex, it is important to understand the patterns of natural hair growth in this area. There can be a so-called “S” pattern, which describes a clockwise whorl, a “Z” pattern, or counterclockwise whorl, a combination of the two with two adjacent whorls, viz., a “SS” or a “SZ”, or a diffusion pattern. In Ziering’s study of these types of patterns, he did not find a “ZZ” pattern that occurred.2 He also found a higher incidence of the diffusion pattern in African-Americans and women. Evaluating a patient’s vertex for existing hairs that remain or tiny vellus “ghost” hairs can help guide vertex creation and placement of the central aspect of the whorl(s). If a patient does not have any whorl left, it is better to place a single whorl off-center, precisely, on the side of the hair part with the arc matching the hair part, e.g., if the patient parts his hair from left to right the whorl should be designed clockwise on the left side of the head so that the upper limb of the vertex arc matches the hair part and follows the part.
