This video clip showing two-dimensional transesophageal echocardiography (intraoperative). Mitral valve vegetation. The arrow points to a huge mitral vegetation imaged in the 5-chamber view. M-mode study shows the characteristic shaggy appearance of the vegetation. Severe mitral regurgitation (MR) with systolic backflow (arrow) in the left upper pulmonary vein (LVPV) was noted during color Doppler guided pulsed wave interrogation. This necessitated going back on bypass (2nd bypass) and replacing the mitral valve with a prosthesis. Subsequently, only mild valvular MR was noted.
