The severity of TR can be underestimated if one does not take into account laminar flow signals moving in the same cardiac phase with the turbulent flow signals. Also note that the Nyquist limit is kept high at 56 cm/s which could also lead to underestimation of TR. It should be between 40 and 50 cm/s for optimal evaluation of regurgitation severity.
