This video demonstrates to sitting on a chair which has adjustable seat height, though most of the surgeon seats are two-and-half feet above the ground. The chair has a nice cushioned back support and a bar to rest the surgeon’s feet. The foot paddles of electrosurgical generators are kept on a wooden plank which is also adjusted for the easy approach of the surgeon. So, now the surgeon sits comfortably through the procedure with the feet nicely rested on a foot bar of the chair and back well supported by a cushion. In the beginning, used to sit only at the times when the body needed a more resting posture, due to fatigue. But gradually, found that sitting all through the procedure right from trocar insertion till the end, is also a very nice idea to complete the entire procedure while sitting. The surgeon can easily get up, if at all, required to approach the patient from a standing position. Suturing in a sitting position is only possible if the surgeon is used to ipsilateral suturing. The chair is positioned according to surgeon’s preference of the side. Most gynecology laparoscopic surgeons are used to standing on the left of the patient, so, the chair is positioned on the left side with surgeon adjusting the angle of approach to the patient and table according to patient’s weight, abdomen fat thickness and patient’s surgical demands. Usually position the chair so as to approach the table by a little side angulation so that our legs and feet are not exactly hitting the table. The exact technique of suturing in the sitting position does not differ from the standing position much. The same needle holders, suture materials and other graspers are required. The only thing that changes is the comfort of the surgeon. The chair height is adjusted according to the surgical field approach which may be different from case-to-case. Main determining factors are patient’s weight, built and fat distribution around the buttocks and pelvis. Patients should be brought closer to the lateral edge of the table so that the surgeon’s approach angle improves. Rest of the surgical suturing progresses as in a routine standing posture. Chair laparoscopy says that full surgery can be carried while sitting and even suturing, if the surgeon resorts to ipsilateral suturing can be carried out with comfort.
