In this video case of 28-years-old female patient present complaints is primary infertility for two years. Ultrasonography: normal uterus and bilateral adnexa. This video demonstrates the procedures of salpingo-ovariolysis. Patient laid in modified lithotomy position under GA. Four ports laparoscopy done, all 5 mm ports except camera port which is 10 mm. Flimsy adhesions noted between right ovary and back surface of uterus in pouch of Douglas. Using cold scissors all these adhesions are cut. Adhesiolysis must be started as close as possible to the uterus by inserting forceps or a suction irrigation cannula into the tubo-ovarian fossa. The instrument allows the adhesions to be tensioned, which are then sectioned either with the harmonic or by electrosurgery or by cold scissors. Flimsy adhesions also noted between right tube and ovary. The adhesiolysis performed to make free the tube from the ovary in its whole length using low setting bipolar coagulation. Patient will be managed with infertility treatment in coming cycles.
