In this video case of 33-years-old female patient, present complaints menorrhagia for six months and primary infertility. On inserting the scope, there was an anterior wall myoma of 7 × 7 cm. Both tubes and ovary were normal. Diluted vasopressin (20 units in 200 mL NS) was injected over the capsule of myoma at the same point till complete blanching. With harmonic ACE a transverse wide incision was given over the most prominent part of myoma. A myoma screw was applied over the myoma. With cold scissors bridges between myoma and capsule were cut and myoma dissected out from its capsule. Hemostasis of the capsule achieved with bipolar energy. Similarly another small myoma at the left cornua was enucleated. Myoma bed was repaired in two layers with vicryl no 1-0 avoiding any dead space. This patient also had polycystic ovaries which were drilled with monopolar needle. Enucleated myoma was removed by electromechanical morcellation. Thorough suction irrigation and lavage done. Patient was managed with active infertility management.
