This video case of 29-years-old female married for 7 years she present complaints secondary infertility. Sonography shows ill-defined gestation sac in the lower uterine segment, uterine cavity is empty. Presumptive diagnosis: patient had complaint of two months amenorrhea with continuous off and on bleeding. Port placement: patient laid in modified lithotomy position under GA. Four ports laparoscopy done, all 5 mm ports except camera port which is 10 mm. Surgical procedure: on inserting the 0°, 10 mm scope, there is some amount of blood in the peritoneal cavity and adhesions of omentum over the lower segment. After removing adhesions a bluish bulging swelling noted on the lower uterine segment. The diagnosis of cesarean scar ectopic pregnancy was made using alternate graspers and harmonic ACE adhesiolysis carried out. An incision made on the most bulging part of the bluish swelling and chronic ectopic bluish chronic contents sucked out. The base of the ectopic pregnancy is cleared and hemostasis achieved. Then the uterine defect was closed with 2-0 vicryl in two layers taking care to close the peritoneum meticulously. Follow-up: HPE report confirmed ectopic gestational contents. Patient was advised to avoid pregnancy for 6 months and also advised to take early hospital visit in next pregnancy. The patient was followed with further infertility treatment.
