This video is a case of 36-years-old lady presented to with long-standing primary infertility of 20 years. Ultrasonography showed bicornuate uterus, rest of the infertility evaluation in both partner was normal. Presumptive diagnosis: complete septate uterus with single cervix with single vagina. Recurrent Pregnancy loss 13% infertility 3.5%. Reproductive potential following the unification procedure and improved chances after obtaining a good cavity were discussed and she was taken up for metroplasy. Surgical procedure: on inserting the 10 mm telescope, bicornuate uterus seen with both ovaries adherent to back surface of uterus. Endometriotic implants also found under the ovary which are fulgurated. Both the ovaries made free from the uterus. Linear incision given over the both cornua starting just beneath the fallopian tube attachments. Both the incisions are continued medially up to the meeting part of both cornua. The uterine cavity opened, manipulator tip is visible in cavity. Now the ipsilateral suturing through left sided ports started to unite the both cornua. In first layer endometrium and one-third inner myometrium is sutured from posterior to anterior. Second layer involves outer two-third of myometrium. In third layer serosa is approximated. Few extra stitches are taken to enhance perifundal region integrity. At the end good size single uterine cavity obtained. Complete hemostasis achieved.
