This video showing managing macula-threatening TRD. In case of macula threatening TRD, after TA-assisted vitrectomy, PFCL injected to stabilize macula. Surrounding fibrovascular membrane peeled with the suction of cutter. Iatrogenic break can be formed during membrane removal. More PFCL then to be injected to make posterior pole more stable. Overlying posterior hyaloid removed. Bleeders are secured with cautery. Fluid-air exchange started and PFCL removed. Breaks are laser barraged and PRP done. Silicone oil injected as tamponade.
