This video clip shows a case of advanced proliferative diabetic retinopathy with elevated macular tractional detachment. TA-assisted vitrectomy done. Posterior hyaloid was tightly adhered at the posterior pole and PVD seen at periphery. An opening in posterior hyaloid made at periphery and cutter introduced in subhyaloid space. Posterior hyaloid seperation started from ‘outside-in’ manner. Segmentation of the membranes done. Thick fibrous membrane delamination done by ‘lift and cut’ technique and finally bimanual membrane separation, segmentation and delamination completed. PFCL injected, fluid-air exchange started to settle retina through the break, laser completed and silicone oil injected.
