A case of post-traumatic blood stained posterior capsule with old RD with advanced PVR changes on ultrasonography. Lens aspiration was done and foldable PCIOL was injected. The time of primary posterior capsulectomy, detached retina was found to be attached with PC. Vitreous hemorrhage was cleared and anterior PVR changes were released partially using proportionate reflux. Nearly, 270° relaxing retinotomy had to be performed to remove anterior PVR components. PFCL was injected to flatten the retina. Laser barrage was done all around. Silicone oil was injected in one hand and PFCL was removed with other hand.
