This video demonstrates a case of dropped single piece PCIOL because of large PC rent. 23G cannula was introduced and scleral tunnel was made. Vitrectomy was started and injected triamcinolone. PVD induction was done with the suction of the cutter and residual anterior vitrectomy was done. Dropped PCIOL was free of any vitreous adhesions. Now PFCL was injected to float up the IOL. Haptic was held with end gripping forceps and brought at pupillary plane. Anterior chamber entry was done and IOL was removed through scleral tunnel. PFCL was removed. Last drop of PFCL was removed with silicone-tipped backflush needle. Vitreous base was checked. Viscoelastic were injected behind the iris to form sulcus. Multipiece PCIOL was placed in sulcus, keeping the haptics vertically as nasally posterior capsule support was absent. Centrally posterior capsulectomy was done. All cannulas were removed.
