This video presents dropped PCIOL removal by active suction. There was dropped PCIOL with intact capsulorrhexis margin with inferior zonular dialysis. Vitrectomy was started to remove vitreous attachments surrounding the PCIOL. Triamcinolone was injected to remove residual vitreous attachments. Central capsulectomy was performed in posterior capsule. Vitreous base was checked. Now active suction was applied over IOL optic with flute needle and IOL was bring at pupillary plane. IOL haptics and optic was prolapsed through the PC opening. Foldable IOL was explanted out by extending corneal tunnel. A multipiece IOL was implanted over capsulorrhexis margin in the sulcus and 23G cannulas were removed.
