This video demonstrates surgery for macular holes. Three pars plana 27-G ports were introduced. Phacoemulsification was completed and put foldable posterior chamber intraocular lens (PCIOL). 27-G twin Chandelier’s light was introduced. Now vitrectomy was started. Triamcinolone was injected and residual TA particles were removed with the suction of 27-G cutter. Brilliant Blue G dye was injected in saline and waited for 2 to 5 minutes. The surgeon used 27-G membrane scrapper and 27-G membrane pic forceps for ILM peeling. Excess dye was removed with the suction of a cutter and dye stained ILM peeling was started creating a flap by 27-G membrane scrapper and ILM peeling was completed in an onion-peel manner by 27-G end gripping forceps. Vitreous base was checked for any break. Fluid-air exchange was done. 27-G valved cannulas along with twin light sources were removed. Long-acting gas was injected at the end of surgery.
