A 25-G valved cannula with trocar-cannula introduce directly to introduce Chandelier light. 27-G vitrectomy started and vitreous surrounding the inferior horse shoe tears removed and margins trimmed. Triamcinolone particle injected. PVD was already induced and residual TA particles removed with the suction of cutter. Now heavy liquid PFCL injected to stabilize the posterior pole. Vitreous-based excision started by external scleral depression. Fluid-air exchange started and anterior SRF drained. Laser barrage done around the inferior breaks as well as 360 degres. PFCL removed and silicone oil injected through the 25-G port with endoilluminator in right hand.
