This video demonstrates a case of progressive tractional retinal detachment involving macula with granuloma formation over optic nerve head. While doing vitreous base excision, a granuloma detected inferiorly at pars plana. After removal of adhesion, a worm detected and it started moving actively. A foreign body holding forceps was initially tried to hold the worm, then an end gripping forceps used to separate it from the vitreous base. But accidentally, it fell down inside. It was removed by forceps from the detached retina. Optic disk granuloma removed by forceps, followed by intravitreal spatula to separate it from the adjacent macula and disk. Then cutter was used to trim the granuloma. Retina settled by internal drainage and barrage laser done around the retinotomy, inferior break and 360 degrees periphery and silicone oil injection given. The worm was examined and diagnosed as Gnathostoma spinigerum.
