This video demonstrates complications during posterior vitreous detachment induction with attached retina. It was an epiretinal membrane removal surgery. During TA-assisted PVD induction, peripheral break formed and shallow subretinal fluid appeared. Vitreous, surrounding the break checked and vitrectomy started in areas away from the break using low suction and maximum high cutting rate. Break cauterized. Peripheral vitrectomy completed. Trypan blue dye injected under air to stain epiretinal membrane. Epiretinal membrane removal done and fluid air exchange done and peripheral localized SRF drained through the break. Barrage laser done around the inadvertent break. Postoperatively retina was stable with improvement in macular anatomy without epiretinal membrane.
