This video sowing stepwise approach to manage intraocular bleed cautery. In a case of full thickness macular hole with tightly adhered epiretinal membrane (ERM), during removal of ERM, suddenly bleeding started because of firm attachment of ERM with blood vessels. ERM removal completed in presence of hazy media. Cautery of the bleeder tried. Clotted pre retinal blood removed and true bleeder noted and cauterized. BBG dye injected in saline and ILM peeling started. Again bleeding started from capillaries due to traction during ILM removal. IOP kept high. ILM removal completed with the suction of cutter. Fluid-air exchange done and fluid removed from the base of macular hole.
