This video demonstrates intraoperative retinal break with retinal detachment. A case of macular epiretinal membrane (EPM). After PVD induction, trypan blue dye injected under air. But macula appeared boggy and a fluid tract noticed extending towards inferotemporal periphery. The whole appearance was assessed again. ERM removed from the boggy macula. Suddenly bullous retinal detachment appeared and inferior temporal peripheral breaks noticed. The detached retina advanced rapidly and became retrolental. PFCL injected to settle retina and peripheral vitrectomy done. Fluid-air exchange done to settle retina and barrage laser done all around. Silicone oil injected. At the end of surgery, the infusion cannula was realised to be subretinal.
