This is patient of cholesteatoma. Socket drilled in place of long process in the body of incus. Groove on short process and socket in body of incus. Placement of incus as interposition graft between handle of malleus and stapes head. This assembly is the closest to the original ossicular chain. While shifting and placing the incus graft, we use a small-sized suction tip in one hand and sickle knife in other. Socket in body of incus is about to fit on head of stapes while the groove in short process will go under handle of malleus. Tragal cartilage graft (TCg) in place. Lifting of temporalis fascia for checking the final placement of incus interposition. Always look for final placement of ossicle graft by lifting temporalis fascia.
