Atlas of Neurology—A Case-based Approach
by Satish V Khadilkar, Kalyan B BhattacharyyaA 38-year-old male presented with clicking sound in his ears for the last three months. Identify the condition. This is usually caused by ischemic lesions of the brain stem or cerebellum, which disrupts the dentate-rubro-olivary pathway. Characteristic MRI finding is hypertrophy of the inferior olivary nucleus involving the Guillain-Mollaret triangle which consists of the red nucleus and the inferior olive of the same side and the dentate nucleus of the cerebellum of the opposite side. This is one degenerative condition where the anatomical substrate, the inferior olive, undergoes hypertrophy.
