Vascular parkinsonism or lower body parkinsonism accounts for about 5% of all cases of parkinsonism. It results from chronic vascular insufficiency in brain, leading to ischaemic insults and thus it is a variety of secondary parkinsonism. Classical signs of Parkinsonism, like rigidity, bradykinesia, tremor, etc. are characteristically absent here and the brunt of the disease falls on the lower limbs, and broad based ataxic gait, upper motor neuron signs and subcortical dementia are the predominant features. The gait is often referred to as Dalmatian gait, following that of the named dog. Imaging shows multiple infarcts and leukoaraiosis while treatment to laevodopa is not gratifying.
