There is incessant tremulous movement of the lips and chin in both the patients. This is a complication of chronic anti-psychotic drug therapy and the perioral tremulous movements are usually in the vertical plane in the range of 5 Hz. This is a tardive syndrome since it occurs months after therapy. Drugs like haloperidol, chlorpromazine, fluphenazine, pimozide, as well as the newer anti-psychotics like, clozapine, olanzepine, quetiapine, aripiprazole and risperidone are the usual offending agents. It is treated with incremental dosage of anticholinergics. This is a specific syndrome and affects only the buccal region in a highly stereotyped manner. Substantia nigra pars reticulata has been implicated as the anatomical substrate for this condition.
